- Family member information (names, relationships, and phone numbers)

We collect your personal information for the following purposes:
- To process your patient pre-registration for our clinic
- To contact you when the clinic opens and schedule your initial appointment
- To provide Korean interpretation services if requested

By submitting the pre-registration form, you consent to the collection, use, and disclosure of your personal information as described in this policy. You may withdraw your consent at any time by contacting us at info@swiftmedicalclinic.com. Withdrawal of consent may affect our ability to provide certain services.
5. Disclosure of Information
Your personal information is sent to our clinic email (info@swiftmedicalclinic.com) via a secure email delivery service (Resend). We do not sell, trade, or otherwise transfer your personal information to third parties. Your data may be processed by our email service provider solely for the purpose of delivering registration notifications to our clinic.
6. Data Retention
We retain your pre-registration information until you become an active patient, at which point it will be incorporated into your patient file. If you do not become a patient, your information will be securely deleted within 12 months of collection. You may request earlier deletion by contacting us.

Under PIPEDA and Alberta PIPA, you have the right to:
- Access your personal information held by us
- Request correction of inaccurate information
- Withdraw your consent to future use of your information
- Request deletion of your personal information
8. Security Safeguards
We implement appropriate technical and organizational measures to protect your personal information, including secure transmission via HTTPS, bot protection mechanisms, rate limiting to prevent abuse, and HTML sanitization to prevent injection attacks.
